The office of Neil R Woods, DDS, PA has been closed permanently
- If you need your dental records, please contact the office by email at staff@neilrwoodsdds.com (email preferred) or by phone at (410) 647-1800.
- Leave your name, email address, mailing address and phone number. We will then forward a Release Form to you for your signature.
- YOU MUST RETURN YOUR SIGNED RELEASE FORM TO US BEFORE WE CAN RELEASE ANY REQUESTED INFORMATION. You may return the form electronically. Upon returning your release form, please also indicate the form and format in which you request the records to be released.
- There will be no charges for records delivered to you electronically. Unless you have a current credit with Dr. Woods’ office, there will be a charge equal to our costs for copying or CD burning. In addition, there will a charge equal to any mailing costs we incur. Upon receiving your request and signed Release Form, we will advise you of any costs you must pay. Physical records will not be sent until the required costs are paid.
- Please be advised that, due to the COVID-19 crisis and state of emergency declared by the Governor, in-person review or pick-up will be delayed until the release of such restrictions and, then, will be available by appointment only. We will provide further information once the applicable social-distancing guidance has changed.
